The Opportunity

As a Senior Community Care Coordinator, you’ll be at the forefront of shaping better lives for the community. You’ll collaborate with clients, their families, and healthcare providers to create personalized care plans that focus on chronic disease management and well-being. This is more than just a coordination role—it’s about becoming a trusted partner and advocate for those in need, ensuring that care is high-quality, seamless, and client-focused.

About You

  • A relevant Primary or Allied Health qualification (Registered Nurse preferred)
  • Proficiency in systems such as E Tools, My Aged Care, DEX Data Exchange, GeoOp, Medicare, & Proda
  • A strong foundation in care coordination and experience with community aged care services
  • Thorough knowledge of Aged Care and NDIS legislation
  • Leadership skills that inspire teams, paired with outstanding communication and collaboration abilities
  • Experience in working with complex health scenarios and managing multi-disciplinary teams
  • Ability to juggle multiple priorities with ease
  • Current Disability Services Positive Notice Card or Working with Children Check(or ability to obtain)
  • Valid Queensland Class "C" Driver’s Licence
  • National Criminal History Check and up-to-date vaccinations (or ability to obtain)
