POSITION
Full-time position available for a Community Worker, Cairns, 4870, QLD.
SKILLS
You must have at least a relevant Diploma and at least 12 months of relevant full-time work experience.
RESPONSIBILITIES
As a Community Worker, you will provide support to individuals and families within the community facing challenges related to age, disability, health or social inclusion. This role focuses on advocacy, service coordination and community engagement, ensuring individuals have access to the resources they need to live independently and thrive. Additionally, you will play a key role in planning and delivering community programs, ensuring that services are efficient, sustainable and aligned with organisational objectives.
DUTIES
- Build and maintain positive relationships with community members, local organisations and service providers. Facilitate access to essential resources such as healthcare, housing and employment services to support individuals' well-being and independence.
- Act as a liaison between individuals and the services they require, advocating for clients to ensure they receive appropriate care and resources. Help clients navigate complex systems such as healthcare, government services and support programs.
- Collaborate with clients and teams to develop and review personalised care plans that address the needs of individuals. Ensure that these plans align with broader organisational goals and support social inclusion, independence and overall quality of life.
- Contribute to the development, implementation and evaluation of community-based programs aimed at improving social participation, health and well-being. Ensure that these programs meet the needs of individuals and are aligned with industry standards and best practices.
- Oversee the efficient use of resources related to service delivery, ensuring the effective allocation of staff, budgets and facilities. Monitor operational activities to ensure that services are delivered effectively and in line with organisational goals.
- Ensure that all community-based services meet relevant regulatory standards and quality requirements. Participate in regular audits and assessments and contribute to the continuous improvement of processes and practices.
- Identify and manage risks to the safety and well-being of individuals in the community. Develop and implement risk management strategies, ensuring compliance with safety protocols and addressing incidents promptly and effectively.
- Provide informal leadership, support and mentorship to team members. Participate in the ongoing development and training of staff, ensuring the team is equipped with the necessary skills and knowledge to deliver high-quality care.
- Engage with stakeholders, including community groups, local government agencies, families and other service providers, to create a network of support for clients.
- Represent the organisation at external meetings, advocating for the needs of the community and individuals.
- Assist in managing budgets related to service delivery, ensuring financial sustainability.
- Help monitor financial performance, reporting on resource usage and identifying opportunities for cost savings and more efficient service delivery.
- Maintain accurate records of client interactions, care provided and progress toward care goals.
- Prepare regular reports for supervisors and funding bodies, ensuring documentation is kept confidential and in compliance with privacy regulations.
