CCS Regional Coordinator – Community Care Services


The Opportunity

A Regional Queensland Council is seeking an experienced CCS Regional Coordinator to lead the delivery of Community Care Services across multiple communities.

Reporting to the Director Corporate and Community Services, this role provides leadership, support and strategic direction to a multidisciplinary team delivering aged care, disability support and community-based services. You will play a key role in ensuring clients receive high-quality, person-centred care while maintaining compliance with funding, accreditation and legislative requirements.

About the Role

You will lead the Community Care Services team to deliver high-quality services that support clients to remain independent, connected and engaged within their communities.

Key responsibilities include:
• Leading, mentoring and supporting Community Care Services staff
• Managing service delivery across aged care, disability and community support programs
• Coordinating client assessments, care plans and service reviews
• Ensuring compliance with Aged Care, NDIS and quality standards
• Building strong relationships with clients, families, health professionals and stakeholders
• Supporting funding applications, reporting and program management
• Monitoring service performance and driving continuous improvement initiatives
• Managing operational budgets, resources and service outcomes
• Representing Community Care Services within the broader community and sector networks
• Promoting a positive, collaborative and client-focused culture

About You

You are a compassionate and capable leader with experience within aged care, community services, health or disability sectors and a passion for supporting positive client outcomes.

You will bring:
• Relevant Primary Health, Allied Health or Community Services qualification
• Demonstrated leadership experience within aged care, disability, health or community services
• Strong understanding of Aged Care and NDIS frameworks
• Experience managing staff, stakeholders and service delivery outcomes
• Strong communication, coaching and relationship-building skills
• Experience developing care plans and supporting client-centred service delivery
• Ability to manage competing priorities and work autonomously
• Current Queensland C Class Driver Licence
• Current First Aid and relevant screening clearances (or ability to obtain)
