Community Care Coordinator

The Opportunity

An Aboriginal Shire Council is seeking a Community Care Coordinator to help Elders and community members access respectful, reliable support that enables them to live well and independently.

Based in remote Cape York, this is a relationship-focused role requiring cultural awareness, sound coordination skills and a genuine commitment to community-led service delivery.

You will work closely with clients, families, care workers, service providers and community stakeholders to keep services responsive and well organised.

About the Role

You will coordinate culturally safe community care services and support the consistent delivery of practical, person-centred assistance.

  • Coordinate services for Elders, people with disability and other eligible community members
  • Work with clients and families to understand needs, goals and service preferences
  • Assist with care plans, service reviews, referrals and follow-up actions
  • Coordinate care workers, schedules and day-to-day service priorities
  • Build effective relationships with health providers, agencies and community organisations
  • Monitor service quality and respond promptly to gaps, concerns and changing needs
  • Maintain accurate client records, service information and program documentation
  • Support reporting, compliance and continuous improvement activities
  • Promote culturally safe, respectful and person-centred service delivery

About You

You will combine practical coordination ability with patience, cultural sensitivity and a strong commitment to community wellbeing.

  • Demonstrated experience in community care, aged care, disability support, health services or a related field
  • Ability to build culturally safe and respectful relationships with Aboriginal and Torres Strait Islander peoples
  • Understanding of person-centred care, client choice and community-based service delivery
  • Experience coordinating services, workers, referrals or client support activities
  • Strong communication skills with clients, families and service partners
  • Ability to maintain accurate, confidential records and meet reporting requirements
  • Calm judgement and the ability to manage competing priorities
  • Capacity to work independently and adapt to the realities of a remote community
  • A relevant community services, aged care, disability or health qualification will be highly regarded
